I will do OA IN NYC CONSULATE in January

My pensions and income go monthly into my checks ng account and shows more than 0 equivalent

Questions:

When do I need to get bank statement? Must it be bank guaranteed? If so, what is it , a stamp or cover letter?

Medical certificate from any doctor stating no diseases listed on the link , elephantitis ect

Police report is just statement stating no criminal action and from local police station where I live in NYC

Anyone have opinion as to time it takes to get

Medical certificate for OA visa is from USA or Thailand ( not clear in requirements ( but I assume USA)

Police report from where resided before ?

I lived in NYC most of my life but also Philadelphia for 2 years
