I’m in California and was in a similar situation. I just provided my incorporation documents from the state. There is also a state website where you can get a download pdf copy showing that the llc is active . I sent that in. For the last part you basically need a company letter stating you’re allowed to work remotely etc . There are a few people showing examples on YouTube.
Anonymous participant 804 it’s around $100 or so for me but only covers medical expenses over $10k from what I remember so pretty much 99 percent of the time would be paying out of pocket but better than nothing or ended up with a 100k bill in a medical emergency
