What is the address you sent it too in Bangkok?

Did you just complete the TM47 for and sign it, and include only that in the mail, or did you also include a copy of passport or anything else?

Can you kindly confirm that for the First 90 days of my dtv I physically need to go into immigration office in Bangkok to complete the TM47 and physically hand it in, or can I do this all online? I am unclear because of mixed information out on the internet.
