TLDR : Answer Summary
The requirement for a TM30 when applying for a visa exempt extension in Phuket is unclear, with mixed reports from the community. However, several commenters indicate that for the three immigration offices in Phuket processing visa exempt extensions, a TM30 is not required. It's noted that some offices may require it for other applications, like a residence certificate, and that hotels are obligated to report guests within 24 hours. Some users have successfully used their hotel booking confirmation as an alternative in the past.

Been there in 2020 they wanted to have something from me - but since I didn't know about the TM30 I didn't ask my hotel for it but at the office they were fine with me showing the booking on my phone instead
it’s not. A TM30 is not required in Phuket for short term extensions.

For offices that require a TM30 (Phuket would for something like a residence certificate or long term extension) then a print out of a reservation would not suffice.
All hotels are legally obliged to register you within 24 hours of your arrival at their premises. Almost all use the online reporting system rather than trudging down to the immigration office everyday to report.
Whether all hotels actually do report you is a different matter.
Actually the majority will actually do the filing (part of their license insists on it) but they probably just can’t be arsed to search for your filing amongst the 30 or so other guests that checked in that day.
