TLDR : Answer Summary
The TM30 form may be required for a 30-day extension of a Tourist Visa depending on the local immigration office. In general, it's advisable to have the TM30 receipt, especially if staying with a Thai national; your girlfriend can assist with the reporting process. However, there are exceptions, as noted that it may not be required in Bangkok.

Depending on your local immigration office, many require you to show TM30 receipt for a 30days tourism extension. Your girlfriend may be able to report you with her ID and lease contract (or title deed if she owns it).
