The need for a TM30 form when applying for a 30-day visa extension in Thailand varies by immigration office. While some offices like those in Chiang Mai and Bangkok generally require it, others such as those in Phuket and Koh Samui may not. Personal experiences indicate that it's safer to have the TM30 on hand due to inconsistent enforcement of the rules across different locations.

It all depends on the I/O you’re dealing with…my advice is simple its better to have and not need then to need and not have

Completely depends on the office you are going to use. In Phuket it’s not needed. In Chiang Mai it is. In Bangkok it appears to be hit and miss whether you’ll be asked. I’m not sure of the policies of the other 70+ offices around the country.
