Currently in Bangkok, needing medical treatment that may last 6 months. Arrived on 30 day with 30 day extension.
Is there anyway to get to stay longer till my treatment is done now that Iβm here already, or would I have to do a visa run after 60 days .. wondering if anyone may have that knowledge

While talking with your doctor about an extension for medical reasons, you may want to do some research on Tourist Visa medical treatment purposes (or Medical Tourism visa) at the consulate/embassy nearby country in case.
However, reading your post again, if you are able to do a border run, then you can't be unfit to travel ? The medical extension is for emergencies and severe injuries, not elective treatments, that is what the medical visa is for.
Speak to the hospital/doctor who, (if you are unfit to travel), can give you paperwork for immigration that can get you a medical extension. (Up to 90 days at a time).
