just apply for a regular single entry tourist visa
That gets you 60 days when you show up and you can extend that one time for 30 more days. 90 days should be plenty of time to get LASIK then recover enough to fly

they do issue a tourist visa for medical needs of less than 60 days and a Non-Immigrant 90 day visa for medical needs of more than 60 days.
You would have to go to the thai consulate you're going to apply at and see what the requirements are for each one
OR
just get a "regular" tourist visa (for tourism) come in and get the documentation from the treating hospital/doctor for the extension based on medical reasons inside the country after you're here at the immigration office
