 we went into the embassy and submitted paperwork and application. Took 3 days and then we went back and picked up our passport with DTV.

 She is my dependent, so we just needed a copy of our marriage certificate. And she filled out same application form and paid 10,000 baht.

 Don't know about any list of Muay Thai gyms registered for DTV. But my gym wrote letter on company letter head and official stamp showing it is a registered muay thai gym.
