We got out DTV in Jakarta! 3 day turn around time. Here is what we submitted:
* Passport
* Letter from Muay Thai gym (I signed up 1 month membership and the owner created a letter to submit to embassy. For those of you lookin for Muay Thai class in Bangkok, I would highly recommend Tiger Eye Martial Arts.
(www.facebook.com/YourTigerEye) Just 1 month membership got me 5 year DTV and owner is super nice and professional. They also offer yoga classes too if you don't want to do muay thai.
* 1 month bank statement
* Application with 2 passport photos
That was it! I'm not sure with this new evisa thing but we just walked in without an appointment. My wife also received DTV visa with her passport, marriage certificate and application (no bank statement.) I hope this information is helpful!

TLDR : Answer Summary
A user shared their experience of obtaining a Digital Nomad Visa (DTV) in Jakarta within three days. They submitted their passport, a letter from a Muay Thai gym, a month’s bank statement, an application form, and two passport photos. Their spouse obtained a DTV as a dependent without needing a bank statement, contributing only the marriage certificate and application. The discussion also touched on the ease of the application process, the requirements for bank statements, and whether an in-person application is still accepted in Jakarta.
Don't know about any list of Muay Thai gyms registered for DTV. But my gym wrote letter on company letter head and official stamp showing it is a registered muay thai gym.
