? I did that as well. It gives the option for background images to be turned off as well so it cleans up the form, but just want to make sure what it is I am supposed to make a pdf of to carry around with me. :)

I read it can take a few days, but mine was done the next day.

I turned off popup blocking for that page. It did try to block once but fixed it. I think one of the 2 versions in the OP should work?

It's just as I said, it never says "print this" or words to that effect, nor does either of the 2 versions in my OP show the next date due. I can figure that out of course, it's just odd that none of these usual internet/form niceties are included in this process.

I did mine after 5pm so it wasn't approved till the next day.

The 2 screenshots I posted in the OP SEEM to be the page I should print, but just wanna make sure.

At the bottom there are these 2 buttons neither of which are there to actually print.
