TLDR : Answer Summary
For extending a 45-day visa exemption as an American in Bangkok, the recommended immigration office is Muang Thong Thani. Online appointments are required, specifically at Counter K. When you visit, ensure to bring your passport, relevant copies (passport page, previous visa, recent stamp, and departure card), a completed Extension of Stay form, a passport photo, and 1,900 THB for the fee.

Hi Ben. I have looked in the Files section, and seen the list of forms. However I don't see one called "Extension of Stay". I even tried to search it specifically. But no results were returned. Please advise. Thanks!

And as far as the Online Queue website not working, I think their SSL certificate must have expired. So it says "Not Secure". And you can't get through. Hopefully they'll have it fixed by tomorrow.
