You will receive a letter from the embassy along with your passport asking Thai immigration to transfer your visa to the new passport. I just did this about 3 weeks ago.

When I got a new Belgium passport in Bangkok the embassy gave me a letter explaining why I got a new passport. With this letter and my old and new passports Thai immigration transferred the necessary information.

Walk in to Airport immigration. Give them both passports and a full photocopy of every stamped page in the old passport and face page of both. You will get a couple of forms to complete.
Hand everything in. Get it back. Usually around 3 stamps and writing in the front page.
MAKE SURE YOU CHECK EVERYTHING AND EXPIRY DATES ARE ALL THE SAME.
Next extension, just in case take both passports as well.

Sometimes there will be a letter sometimes not. As Australians we personally write the new passport number in the old passport.

Iโm English and had a new passport last year I went to immigration and the lady said I needed a letter from my embassy. When I told her I had just come in on the new one she was advised by one of the senior guys there that I did not need a letter from my embassy. That was Jontiem Immigration. Hope that helps

You cannot transfer a visa, but you can transfer an extension. You would need to take both passports to immigration.
