Take the letter you get with your new passport to an immigration office and they'll put a new 'admitted to stay' stamp in your passport. They wont transfer the visa so you need to keep your old passport untill the visa expires.

Not thailand but I had this issue a while ago with a 10 year Brazilian visa and they said just to keep my old passport, which I guess if it’s lost or stolen isn’t an option?

Your Embassy will provide you with a letter for the Immigration and with that letter you gonna get a new stamp.
