TLDR : Answer Summary
The necessity of the departure card for a 30-day visa extension in Thailand seems to vary based on individual experiences and specific immigration offices. Some users confirm that a copy of the departure card is needed, while others report that it was not requested during their extension process. If the departure card is lost, it is advisable to report it at a police station and then visit the local immigration office to apply for a replacement. Overall, experiences indicate there may be flexibility, but it is recommended to locate the card if possible.
Maybe not at CW but likely accepted at outbound immigration counters. Almost every trip out of Thailand will see at least one person losing their TM6. The IO just gave them a new one to fill it in.
Lynne Elizabeth yes but it will not be the same TM6 number that you arrived with...thus they may ask for a police report before they will accept another... it happened to my friend
Depends on your immigration office, but normal procedure is you visit the police station to make a lost document report after that you visit your local Immigration office and with these police report you apply for an replacement.
