Maybe not at CW but likely accepted at outbound immigration counters. Almost every trip out of Thailand will see at least one person losing their TM6. The IO just gave them a new one to fill it in.

and immigration may not accept a new one, with a different number, without a police report first...

Lynne Elizabeth yes but it will not be the same TM6 number that you arrived with...thus they may ask for a police report before they will accept another... it happened to my friend

Went to Changwattana for a tourist visa extension. Forgot to bring my TM6. They did not ask for it. That's my experience. They may or may not ask you.

Depends on your immigration office, but normal procedure is you visit the police station to make a lost document report after that you visit your local Immigration office and with these police report you apply for an replacement.
