TLDR : Answer Summary
Yes, your friend can still extend his visa for 30 days even if he lost his departure card. He can obtain a replacement departure card at any immigration office, usually for a fee (though in some cases, it may be free), and it's advisable that he also checks if a police report is necessary for the replacement.

Replacing departure cards at the immigration office is free

Although they can request you go get a police report before they give you a new one if they want to.

, It happens a lot, and its not really considered a big deal. I lost mine back before i retired and didnt even worry about it, when i got to the airport to leave, i just asked the friendly Qatar airways lady to give me another, she was happy to do so.

if you lie stating you left it at the hotel and they say go back and get it what are you going to do then?

Just man up and say you lost it, it's not the end of the world and believe me you're not the first foreigner to lose a departure card in the history of thailand ;)
