A few years ago I lost my PP and left the country with a temporary PP without my departure card. Immigration took me aside and started searching for the number in their system of my arrival card, which corresponds with the deparure card, and they told me if they can't find it, I can't board and will have to go to Immigration the next day. They found it, have me another departure card to complete and let me go. But that was then...

Just as Robert and Tod say - I lost mine and on departure asked at check in counter for one and happily given one, no probs at immigration. (This was two months ago)

and you went to the immigration OFFICE before you went to fly out for an extension without a departure card? OR are you talking about the fact you had no problem with the immigration officers at passport control when you took a new departure card before flying out/

if you are needing to do an extension OR anything at the immigration office that requires it you should get a police report BEFORE you show up at the immigration office to get a new card.

IF all you're going to do is use it when you exit the country you can just get a new one at the airport when you leave

You visit the Immigration Office and ask for a new one, some will just give you others will want to see a Police Report of losing this card. Of when leaving Thailand get a new one before departure.
