How do you request for a new departure card? I lost mine, but would need to apply for the covid visa extension.

To request a new departure card (TM6) after losing the original one, you need to file a police report at any police station. Once you have the report, take it to your immigration office, where they will provide you with a new TM6 card, which will have your old number written on it.

You go to ANY police station file a police report saying you lost the original TM6 card, take that report to your immigration office and they'll give you a new TM6 departure card, look up your old number and write it on that card.

you will need your passport to make any police report (except in case of lost passport).

Which police office you need to go depends on the address you are going to use as the possible address you lost your belonging. Go to the police office that serves that address.
