What documents are needed for a 30-day extension on a 2-month tourist visa in Thailand without an entry/departure card?

I have 2 months Tourist visa. What documents do I need to get 30 day extension?

Notably there's no entry/departure card any more, since i was not given one. Also do I need TM 30 receipt if I'm staying at hotel and how do I prove I'm at a hotel.

Probably Bangkok. Will stay Nana hotel. Visa expires 18 October.

then there is a good chance that you will not be asked for a TM30 when you go to Chaengwattana and apply in counter J for a 30 day extension from your 60 day tourist visa entry stamp

BUT

The upside is, believe me you're far from the first foreigner to stay at the Nana, tell the staff you need proof of the TM30 and they'll print off the receipt for you
