How can I manage my US passport and driver's license while living permanently in Thailand without a US address?

yes .. just like USA, you can only move in transit for 90 without acquiring and local drivers license (visa indicates date).

IDP is irrelevant in USA. So, just keep a USA address and account and get a Thai drivers license too.

Thai immigration certificate with a doctor signature of well being and sit thru 6 hrs of Thai training movie at Dept of Land Transportation and take computer based test, depth perception and reaction time test and a driving test.
