i started the process too late. I messaged the agent who sold me the policy and asked him if they could do that. I didn't hear back from him. And next week is so many holidays, I wont get anything done then. Better to take the hit then end up being late

I just went through this at Udon Thani. My visa expires in december but my insurance is good through march. So they gave me an extension until march and I have to go back and renew again

i'm not asking you to answer the question, it seems that this question is incompatible with the long stay visa, which is why i'm sure they have the additional form. My guess for this entry if you are applying for a long stay visa to put long stay here. Or maybe put "See Additional Form"
