FYI: In the US you can apply for a multiple entry Tourist Visa valid for 6 months through the eVisa site Tod posted above, with the same rules per entry. Will give you more flexibility if you need it.
your absolutely right, but there is much more to it than a 5 year entry. Stamps and endorsements took 2 passport pages. You may want to contact with the BOI to make sure your getting all you paid for. James Miller was nice enough to post how above.
Similar, only this guy had an existing visa he hadn't canceled. Very clear you have to do this, the BOI can't do it for you. When I left he was yelling at a supervisor, bet it didn't end well for him:)
