I have a multiple entry tourist visa. At what day should I get an extension and how does it work. For instance if I get an extension at day 45 does it only extend to day 75 or does it extend to day 90.

With METV (and probably all visas) you don't actually extend the visa, you extend the stamp (permission to stay). So if you stamp-in for 60 days, you can extend that stamp by 30 days, given you 60+30 days per stamp/entry. After 90 days you must leave the country and stamp back in for another 60 days, then repeat. You can get close to 9 months on an METV. In theory you can have a full 9 months but depending when you activated the visa (first stamp) and when you finally leave you may loose a few days.
To avoid the Pattaya Immigration office I am choosing to not extend my visa stamp, but rather I will do a border run. In my case I will stamp in for 60 days, at/about day 60 I will travel to Cambodia, have lunch, come back. I may extend my last stamp or just use my METV for 8 months.

Are you aware you can get 8 to 9 months out of the METV visa?? Check the expiry date on the visa, it should be valid for 6 months, if you do a border bounce just before it expires, and after that a final 30 day extension, that gives you 8 to 9 months in total.

I have been in Thailand now for 7+ months on an METV, I did the border bounces at the thai/Lao border myself, with no problems whatsoever, you basically just stamp out of Thailand, cross the friendship bridge on a bus, fill out a simple Lao visa form, you can pay for the Lao visa with Thai baht, its ฿1700, you just need a passport size photo, and a printed pdf of your METV, and obviously your passport, you can leave Thailand at the Lao border and come back the same day if you wish, when you return back to Thailand you just need to fill out the TM6 arrival card. If you don't want to do it yourself, depending on your location, there are border bounce companies that take you to the border and back again, if that would be easier for you.
you don't need to buy a re-entry permit, if you have got a multiple entry tourist visa (METV) when you cross the border back into Thailand by most land borders, you just need to fill in the TM6 arrival card, and present that along with your passport and a printed pdf copy of your METV to the immigration officer, and make sure you check they have stamped you in for 60 days, before you walk away.
Yes that is what I’m trying to figure out. What is needed for the border bounce near the end of the first and second extensions. I’ll search this group for border bounce as well but any information is greatly appreciated. I will be seeking the timing and re-entry procedures necessary for a successful border bounce and also try to understand potential pitfalls.
three days before the expiry date of the visa simply go to a border, the one at Nong Khai into Laos is easy to get to, and exit Thailand. Enter Laos, and simply turn around immediately and come back into Thailand. You have a multi-entry tourist visa so you do not need anything else, and you don't need any agency to help you. It is all straight forward, with no other requirements. You get stamped back into Thailand for another 60 days, that you can extend for 30 more days. Do NOT try to make it complicated because it isn't.

Go to the immigration office you will be doing the extension at, and get a list of requirements, for instance some will require a TM30, some won't, Just do the extension when you have 7 days or less left on your current stamp, because some immigration offices won't allow you to do it earlier than that, the 30 days will be added onto your current stamp end date.
Oh sorry I forgot some offices also require you to show a filed TM30 which your landlord or hotel should provide. I keep forgetting this requirement because Phuket doesn’t need it.
