What should I do if I lost my TM6 DTAC form required for my Non-Immigrant O visa extension in Thailand?

Arrived on a 3 month Non Immigrant O visa ( retired) and now want to extend. Immigration want the TM6 DTAC form as a requirement which has long???? I don't have it anymore and can't get a copy.

buy a re-entry permit for your 90-days stay permit on the Non-Imm-O visa, for 1000 THB. Exit Thailand, fill out a new TDAC and submit it, receive an email with the new TDAC, re-enter Thailand, and there you are. Your Non-Imm-O will still be valid and you will have a TDAC to showStuart *********
Oo Jig The TM7 paper is asking for your TM6 number. Ignore it. If you came in by air you don’t have one. Leave it blank.
