What happens if you test positive while in test and go hotel? Do you get whisked off to hospital/jail/other? Do you get any choice of where you quarantine for the period?

You have to read the small print on your policy. I had a policy with Tune and found that they would not pay the quarantine costs if you had no symptoms of covid.

I've read in this forum it can be 150,000baht+ I can't comment on your insurance, but I'm planning to get AXA, seems to be the most comprehensive
I think you just have to look for asymptomatic cover - the main concern being that if you don't have any symptoms they would leave you in a hospital at your own cost

Hospital stay at your cost. Check with your insurance company if they will cover said stay. Most will not if you found to be asymptomatic and/or made to quarantine on your hotel rather than hospital. The latter will not be covered.

If you’re asking because you are worried about blowing your vacation do what I did. I isolated before I flew. If it’s a return to your home here in Thailand you would fly anyway right?
Yes! We are already planning to do that. If we come in contact with anyone in the 2 weeks prior to flying I don't think we would be allowed to fly anyway, because we would be under home quarantine. Luckily we are in a regional area with no cases, but the state rules are very strict

Oooh, you don't want to find out! Even if you test negative but someone on your plane tests positive get ready for a big surprise! Insurance will try and weasel out of paying anything cause that's what ins. Does.

If no or mild symptoms you could end up in a quarantine hotel. This is not covered by some of the insurances. So make sure you have one that covers covid without symptoms and no hospital stay.

I used tune and saw it did not cover it but went with it anyways. I saw someone using axa who manage to get them to pay for it. So check with them. One important thing with axa was to save the results for the test you do at the airport, they needed that as proof even when the person did a positive test 10 days after arriving.
any suggestions for finding a policy that covers this? I have insurance that covers everything else but want to top up in case we get hit with enforced quarantine without testing positive as close contacts from the plane

I had AXA and they handled everything. Bill was paid before I left the hospital. I don't think I ever saw a bill. Hope that helps. I would recommend AXA to anyone. I was worried about having to pay the bill and fight with them. But it wasn't the case at all.
I tested positive a few months ago and had to spend the next 14 days in a Thai hospital. They never tested me again. I just had to spend my time there.
This was the summer of 2021. I was at a quarantine hotel and tested positive. So they transferred me to Bang Lamung Hospital. It was OK. The staff didn't speak any English. Once my insurance was approved my meals improved, but I still didn't have any choice and it was only Thai. Some meals I eat, other times my wife brought me something to eat. But I didn't have any problems with AXA insurance. Nothing out of pocket.

Benjamin Hays awesome, Thanks. I assume the insurance will pay for this? I'm also terrified that families are getting seperated. Have you heard of this? (It has happened in Aus)
if only you test positive then they will remain in hotel quarantine and you will go to hospital. If a child tests positive, an adult can go with them but I've heard of problems that the adult then has to do quarantine after the child is released, even if the adult is negative. Also, I'm not sure if insurance will pay for the adult if the adult goes with them as they are technically negative. The main issue is there is no consistency. It seems to be different for different places/people so whatever answer you may get will be different if it actually happens.
I understand but the reality is no parent would let their child go on their own. It would be emotionally traumatic for a young child to go to hospital quarantine independently so parents are forced to pay thousands of baht because the insurance not see that it is a necessity.
