TLDR : Answer Summary
If you test positive for COVID-19 and cannot leave Thailand due to medical reasons, you need to obtain a statement from the hospital where you are being treated. This statement allows you to apply for an Extension of Stay based on medical reasons, which can grant you a maximum extension of 90 days.

Sorry but the Extension of Stay based on medical reasons is based on medical reasons. Simply as that, for preempt reasons to stay longer inside Thailand there are other ways and visa's to obtain before you made your way to Thailand.

has correctly replied.

Medical extensions are available and your doctor and hospital know what documentation you need to obtain one from Thai Immigration.

If you are not seeing a doctor you will not get a medical extension for self isolation at home.

If you can not leave Thailand due to medical reasons, you get a statement from the hospital where you get your treatment.

With that statement you can apply for an Extension of Stay based on medical reasons with a maximum of 90 days.

I am

Testing with an approved company not a hospital. I have no symptoms and am reading people with no symptoms are refusing hospital treatment or hospital stays

I have not seen any reports of foreigners who test positive who can not either get a hospital bed or a community quarantine bed. If you test positive, you will go to either a community quarantine center, a hospit-tel or a hospital. Talk to the nurse on duty about what is required to get an extension of stay based on medical reasons.
