It means there was a mistake in your application form (wrong country, issuing agency, middle name, and so on).
but suggest you not apply in Pakistan again. BUT must be sure that the Thai Embassy or Consulate where he does apply will accept the application with a passport of Pakistan.

No, my application was accepted, was called for interview, then subsequently requested some work related docs and travel history, so if information was incorrect, would have cancelled straight away
