Hello, documentation required for application of re-entry permit (Non-B Visa Extension)? TM 8 Application Form, signed copy of passport photo page, signed copy of latest visa extension stamp page, signed copy of TM 6/Departure card? Anything else? Maybe copy of 90 day report and/or TM 30? Thank you.
