I'm assuming you're asking for a single entry re-entry permit that keeps your "admitted to" stamp alive when you leave the country. I DO NOT know if Phuket International is selling the Re-entry permits, however the local immigration office will sell them.

According to

So what proof would I have of filing a TM30 previously?

I've done other re entry before. I just can't remember what docs i provided

You would use one of the following three documents as proof of TM30 filing. If you don't have either one of these three documents, contact your property owner for a print out of one of the three documents.

After looking at my notes, I wonder if a TM30 isn't required, and they have a mistake on Phuket Volunteer's website. You should be able to get a re-entry permit at any immigration office.

If my 90 day registrations recently been done. Do I still need to fill out tm30 form for re entry application.

The TM30 should have been filled out and submitted when you first arrived. The TM30 does not become "old" or "expire"

if you mean a 90days report with "90days registration", it's different from TM30, you may need a TM30 receipt.
