To obtain a single re-entry permit in Phuket, you will need the following documents: a TM 30, a completed TM 8 form for the permit request, your passport along with a copy of the data page, current visa or extension of stay, entry stamp, departure card, one passport-sized photo, and a processing fee of 1,000 THB. It's recommended to check at the local immigration office, as Phuket International Airport may not sell these permits.

I'm assuming you're asking for a single entry re-entry permit that keeps your "admitted to" stamp alive when you leave the country. I DO NOT know if Phuket International is selling the Re-entry permits, however the local immigration office will sell them.
According to

you will need:
- A TM 30
- A completed TM8 form (for the re-entry permit request)
You would use one of the following three documents as proof of TM30 filing. If you don't have either one of these three documents, contact your property owner for a print out of one of the three documents.
After looking at my notes, I wonder if a TM30 isn't required, and they have a mistake on Phuket Volunteer's website. You should be able to get a re-entry permit at any immigration office.
