as a rule once you get your yearly extension of stay inked into your passport when you go to buy a re-entry permit you need copies of the data page of your passport, your new yearly extension stamp, your most recent entry stamp & your TM.6 departure card. Those things a filled out TM.8 application for re-entry permit, a passport sized photo and either 1000baht for a single entry or 3800 baht for a multiple re-entry will get you the permit (y)
