TLDR : Answer Summary
When leaving Thailand on a Non-O visa, you must apply for a re-entry permit ahead of your flight. This can be done at the immigration office or at the airport, taking about 15-20 minutes at the airport. The common requirements include submitting the TM 8 form, providing a photo, and paying a fee of 1,000 Thai baht. It is not explicitly stated whether a return ticket or additional documents are needed, but it is advisable to check with immigration for any further requirements.

I am not sure if Chiang Mai has the reentry permits available. Hopefully others will reply.

Are you connecting with BKK Bangkok airport?

Wise decision to get it at local Thai immigration office.

Safe travels...

get a re-entry permit at the immigration office where you got your visa/extension OR you get it at the airport before you fly out. Allow 15-20 minutes max to get it at the airport depending on how many other people are needing one at the same time

Get the re-entry permit from your local Thai immigration office. Single re-entry permit is 1,000 Thai baht. Here's the common found requirements for it.

It's also available at airports...
