A re-entry permit just means your existing extension will not be invalidated when you leave. You don't get any extra time because you got a re-entry stamp, you're still running on the stamp you have in your passport. If you're not back before your extension runs out then it's gone, whether you have a re-entry permit or not.

Just go to immigration fill out form and hand over cash.

You could also do it at the airport, but better local immigration

go to the immigration office that you use in your area and buy one, a single re-entry is 1000baht or a multiple is 3800baht

Bring your passport, you need copies of your data page, the entry stamp, your current extension, your departure card (TM6) and a filled out TM8 application for re-entry with one passport photo stuck on it

OR

if you're flying out of Suvarnabhumi you can buy one the day you fly out just past security at the International Departure 2 area (single 1200 baht, multiple 4000) and you don't need anything but your passport and proof you're flying out

some offices won't sell you a re-entry permit if you didn't get your extension from them.

nope Suvarnabhumi airport charges 200 baht more than the face value of those re-entries because you need nothing but the money and your passport
