What Documents Do I Need for a Re-Entry Permit on an OA Visa in Thailand?

Hopefully a quick one. I'm going to CW to get a re-entry permit on an OA which is about to expire, but I have got the "extra year" admitted stamp already in my passport. I know I need the following: TM8 form with photo attached, copies of passport data page, visa and last entry stamp, and copy of TM6 card. And 3800THB for multiple entry. Is there anything else I'll need? Thanks for your advice
