Hi, I’m coming to Thailand for more than 30 days and intending to get an extension while I’m here , on the Visa form shall I put the 30 days or put the 45 days I intend to stay even though I haven’t got approval to extend yet. Thanks for any help.

Assuming your passport allows (American, British, Australia etc) A tourist visa is good for 2 months right off the bat. You can also arrive without a visa at all and get 60 days exempt currently (this will probably go back to 30 days exemption sometime this year, so keep an eye on that).

On the TDAC put the actual date you are arriving and the actual date you are leaving.
Not sure what country you are from but you probably have a 60 day visa exemption.
most country are visa exempt for 60 days currently, suggestions are that this may reduce to 30 days at some point in the future. The 60 days can be extended for another 30 days at a local immigration office
