There's no such thing as a 45 day visa on arrival. Visa on Arrival is where apply and pay for a visa when you arrive to Thailand.

You have visa exempt. Every office has different requirements but this is a general list of requirements.

Make an online appointment

you need to bring:

*signed in blue ink copy of passport information page

*signed in blue ink copy of entry stamp

* print out copy of visa (if applicable)

* 1,900 baht

* infinite patience with a smile

Your hotel or residence should have filed your TM 30

Why go early in the morning if you have an appointment? The appointment letter suggests 15 minutes before appointment time

just for infos :departure card doesn't exist anymore . so it's time to maybe update this answer

you're wrong. TM6 is only temporarily suspended for air entry. If you arrive via land or sea you'll receive a TM6 and you must have it with you for an extension.

.I can picture the guy who arrived by plane and is freaking out because he can't find this damned TM6 , that he is supposed to give to get his extension .
