TLDR : Answer Summary
To transfer your visa to a new passport in Thailand, visit the immigration office where your visa was issued, bringing both your old and new passports. If your visa label is in the old passport, it cannot be transferred; instead, keep both passports until the visa expires. If you have an extension of stay stamp, return to the issuing office for transfer, usually at no cost. Make sure to copy all pages of your old passport and check with your bank and driving license authority to update your information accordingly.
IF you have an extension based on "money in the bank" , you also need to prepare your updated bankbook + copy.

Yes you go there, with both passports, and with copies of all the pages in your old passport. They won't want all copies, but you don't know which one's they will want so copying all of them can save a trip to the copy shop.
If you have a visa label in your old passport it can’t be transferred. You just use both passports until that visa expires.
If you have an extension of stay stamp issued by an immigration office then you’ll need to return there to get that stamp transferred. This should be free of charge.

You go to the immigration office that you received your visa or extension at. Note that most embassies no longer issue a letter with your new passport and the immigration office should be aware of this.
