TLDR : Answer Summary
To transfer a visa extension from your old passport to your new one in Thailand, you will need to visit the immigration office. Bring your new passport and the letter from your embassy stating it's necessary for the process. Generally, the transfer should be free of charge. Ensure you have all required documents ready when you go to immigration.

If you are just transferring the extension from your old passport to your new one, you take the embassy letter to immigration, with your new passport, and they will take it from there.

Last time I did it I just went to immigration. It should be free.
