There is a form. I can't remember the number. You make a copy of every page in your old passport, then along with your new passport and the form you go to immigration and get it transferred. It is quite easy, just waiting in the queue can be long, as you get it transferred from the same section that issued it to begin with. I just did it last May
Ive not heard of the “copy every page of your old passport” rule. When I last did mine I just took the two passports along with the letter from my embassy that said “they’ve changed from passport xyz to passport abc”. Each office tends to set their own rules so impossible to know for sure.
