TLDR : Answer Summary
To extend your 45-day tourist visa in Thailand for an additional 30 days, you need to visit the Laksi Immigration Office located at IT Square Laksi Plaza, near Lak Si station of the SRT Dark Red Line. It is recommended to book an appointment online, although it is not mandatory. Ensure you have the TM30 form, a printed copy of your passport including the arrival stamp, one passport photo, and a pen. You will need to fill out three forms at the office. It's wise to ask your hotel for assistance with the TM30 form, as they can provide the necessary documentation.

100 % ask for the TM30. I got asked for mine this morning at IT Square Laksi, and they were turning people away who didn’t have it. Wherever you are staying can do it online for you, and print it out for you. Also take a printed copy of your passport and the arrival stamp page in your passport. Take a pen as well and 1 passport photograph.
Floor 3 at IT Laksi Square. You will be asked to fill in 3 forms.
Someone will check your forms, before you see the immigration officer.
If you need any printouts or passport photos right next to the immigration office are two photo and copy places open the same hours as immigration.

We can confirm it is NOT 100%, but depends on your history of traveling/staying in Thailand.

You should ask your hotel for a print out of the TM30. Hopefully they will know what you’re talking about and just print it out and hand it to you.

You are going to go to IT Square Laksi Plaza temporary office, just next to Lak Si station of SRT Dark Red Line.
You will go here.
