My extended non-O spouse visa expires on 15th May 2025.
I managed to get an online appointment on the 21st of April.
Is this considered too late to extend?

TLDR : Answer Summary
The individual was concerned whether their online appointment on April 21 to extend their non-O spouse visa, which expires on May 15, 2025, was too late. Comments clarified that as long as the application is submitted before the visa expires, it is within the acceptable timeframe, and April 21 is considered an appropriate time to apply.

I think you are misunderstanding the "window of opportunity" you have to apply for a new extension.
At MOST offices you can apply for a new extension when you have 30 days or less left on your stamp <- that means any time from a month before the extension expires right down to the very day it expires.
As long as you're applying before the current extension expires (which you say is May 15th) you're fine.
