I had the same issue when we came Jan 1, I had a hard time completing the form, it was so frustrating! But I managed to resolve it by clearing my browser cache and cookies, maybe that'll work for you? Also, waited until our stopover in Doha & completed with no problem.

I am in Bangkok now and have realized that it can be as cheap or as expensive that you want or allow it to be. Having a great time eating out everyday or not if I want to and the cost is very economical

Thank you but I am well aware that I cannot use for business or banking transactions. But, I have run across a lot of folks that are unaware of WhatsApp to connect with family & friends internationally without paying cell.

Use WhatsApp to make international calls as long as you have wifi it is free. I only pay for calls calling the US if it is an emergency situation. (But I am not working remotely, fully retired)
