I entered with a doctors note on letter head, signed and stamped. Had a little bit more detail than yours, no one gave me any issues.

It does need the date on it though.. Since it has to be within 72hrs. I don't see that in the picture.

yeah... There been some people in the groups have a hard time getting it extended and at least one that could not. It is also so inflexible in terms.

As far as insurance, I just used AXA. It's a Thai company, bought online in 5 minutes... Had no issues.

I just entered a few days ago on STV and I do regret not doing a TR and then later switching to another type of visa.

The STV is so inflexible and I hearing a lot of trouble getting it extended... So in the end it might just be 90 days visa.
