Many users agree that a fit to fly certificate from Germany is generally acceptable for entry to Thailand, especially if it includes specific details such as being on official letterhead, being signed and stamped by a doctor, and dated within 72 hours of travel. One user shared their positive experience entering Thailand with a similar document, while another suggested contacting the airline for confirmation. There is also mention that the fit to fly requirement may soon be obsolete, indicating changing regulations.

Mein Hausarzt fand das total lächerlich, hat es ohne Untersuchung unterschrieben 😂😂 und Anfang 2020 hat das sogar noch zur Einreise gereicht -> auch wenn es damals noch keiner kontrolliert hat.

E-mail your airline as they will approve or not

I entered with a doctors note on letter head, signed and stamped. Had a little bit more detail than yours, no one gave me any issues.
It does need the date on it though.. Since it has to be within 72hrs. I don't see that in the picture.
