Mein Hausarzt fand das total lächerlich, hat es ohne Untersuchung unterschrieben 😂😂 und Anfang 2020 hat das sogar noch zur Einreise gereicht -> auch wenn es damals noch keiner kontrolliert hat.

I entered with a doctors note on letter head, signed and stamped. Had a little bit more detail than yours, no one gave me any issues.
It does need the date on it though.. Since it has to be within 72hrs. I don't see that in the picture.
