I added a cover letter for two different sections of documents as the first page. For the proof of location I wrote the cover letter telling them what I was giving them: driver's license, car Insurance ID, and internet bill. For the financial docs I wrote a cover letter that listed the dates of the bank statements and the closing balances of each.
