Did you hand your passport and a pile of money to an agency who went and got a medical extension for you (probably from a province you don't live in from an office you don't use)
In that case you SHOULD be able to go to the immigration office and apply for a 60 day extension (without the embassy letter).
BUT
you should go to the office where you got the medical extension and ask them in person just to be sure :) Sorry but that's about the only way you'll know for certain. I'd go this week and ask them to make sure.
