My husband has a Dr's letter stating he is not fit to fly until October 25th.
I have a letter from the same Dr stating I need to stay and be his carer.
I have 3 questions.
First, what exactly is it that we should apply for?
90 day extension? 30 day extension, is it even called an extension if we are currently on amnesty?
Second question - when do we apply? Is it better to go now or wait until closer to Sep 26th?
3rd question - how difficult is it to get extra time based on medical reasons?

yes...likely saw the doctor on July 25th and got 90 day medical extension.
Going into Thai Immigration later will not change the doctors dates noted on the med cert. So no benefit in delaying getting the medical extension approved at immigration and new stamp placed in his and her passports.

You can apply on any day at your local Immigration office for an Extension of Stay based on medical reasons, which is maximum 90 days, As 25th Oct is less days, you should get permitted up to 25Th Oct,
If the documents are all there and from a genuine hospital it is not difficult, but Immigration has the right to ask for a second opinion from a designated doctor.
