TLDR : Answer Summary
To transfer your extension of stay and multiple re-entry permit from a cancelled passport to a new one, you need to follow these steps:
1. Obtain a letter from your embassy confirming your new passport details.
2. Prepare a special form for transferring your extension of stay.
3. Gather required documents including copies of both the old and new passport ID pages, old visa, extension of stay, re-entry permits, and the embassy letter.
4. Submit these documents to the immigration office when you collect your new passport.
. It comes from Chaengwattana and is actually printed on the application to get your stamp transferred you fill out

There is a special form to transfer you Extension of Stay to your new passport. You need copies of both id pages, old visa, Extension of Stay, re-entry permits And the letter from your Embassy.
I received my new passport in early October. I was planning to get my Extension transferred at the same time, or right before, getting my new Extension on 8 November. Presumably it's ok to get it all done at the same time?

Your embassy should give you a letter to take to immigration when you collect your new passport .
Then you just need to take the papers Robert mentioned to CW along with the letter from Embassy. You need the Thai form "Transfer stamp to new passport" ( I got mine from Samut Prakan immigration website). and it should be good.
My passport was just mailed to my house, with a letter, but do I just show up at chaengwattana or should I do it at an airport coming back into the country or what?
