TLDR : Answer Summary
To marry in Thailand, you need the Affidavit of Eligibility to Marry, which you can obtain from your embassy. It is suggested to visit the US Embassy, where the form will be provided during your appointment. Bring necessary information and cash for any fees, as the embassy does not allow phones inside.
The reason I was asking I am back in the States for a funeral and thought I would go down to the Magistrate and get a form with their letterheads I don't know if that would be acceptable
I searched for this online and I couldn’t find it. I have an appointment at the US embassy this afternoon and I think they give you the form there. They don’t allow phones inside so write down your address and any other information you think you will need. Bring cash for the fees.
