The reason I was asking I am back in the States for a funeral and thought I would go down to the Magistrate and get a form with their letterheads I don't know if that would be acceptable

I searched for this online and I couldn’t find it. I have an appointment at the US embassy this afternoon and I think they give you the form there. They don’t allow phones inside so write down your address and any other information you think you will need. Bring cash for the fees.
