What documents do I need to apply for a Certificate of Residency in Chiang Mai if I lack a lease agreement?

Certificate of Residency question.

I live with my girlfriend at her house in Chiang Mai. Have TM30 for that address.

No lease agreement, not on the house book and not named on utility bill's. Almost invisible ;).

Have everything else apparently required except a landlord/lease agreement.

Would a note from her and a signed copy of her Thai I.D. be sufficient? Are they charging for these now?

TLDR : Answer Summary
To apply for a Certificate of Residency in Chiang Mai without a lease agreement, you do not need a landlord lease or registration on utility bills. Instead, provide printed proof of TM30, a filled application, a passport photo, copies of your visa/entry stamp, and a fee of 500 baht. You can drop off the documents at the Chiang Mai immigration office and pick them up the next morning.

It’s at Chiang Mai immigration office Level 2. Stairway is left side of the main building. Photo from last year. Should be the same.

You don't need any of that.

Take printed proof of TM30, filled out application, passport photo, copy of your visa/entry stamp (maybe the data page too, I can't remember) and 500 baht.

Drop it off before lunch and pickup the next morning.

if I’m staying at a friends condo, and due for a visa extension what can I do? Will I have to get his receptionist to file a tm30 with my passport? I’m confused

if he owns the condo then he's responsible for reporting you staying there. If he's renting then his landlord should do it.

it's easily done online if they are registered for that system. Otherwise must be done in person at immigration office

if they say they're only offering the 1 month service, wait until there's less people in line and go back up again and ask for express service.

Is the 1 month free? No panic, just to get my expired Driver's Licence taken care of. Using an IDP for now.

I think it's like 20 baht for the 1 month service and they mail it to your address. Up to you if no hurry.

before was 5 or 10 baht for regular Thai post. Take about 1 month. Now they charge 40 or 50 for express delivery. Around 1 week. If my memory is correct.
