TLDR : Answer Summary
When applying for a one-year visa (such as a marriage visa) to Thailand, there is no mandatory health insurance requirement before leaving the UK. However, it is recommended to have insurance in place. Upon arrival in Thailand, the individual will receive a 45-day period to finalize their visa process, during which they can arrange for health insurance if needed.

If he just comes back to Thailand he will get 45 days to sort out the visa process without worrying about the medical insurance and he can get something done once he is here
